When you invite a driver to complete licence checks through Driver Codes, they are presented with a six-step mandate wizard inside the app. The driver must scroll to the bottom of each step before the Continue button becomes active. On the final step they provide a handwritten signature using their finger or a stylus.

Who we are: Driver Codes is operated by RSMT Limited. RSMT Limited is the data controller for obtaining and processing your driving record information as part of this check. We are registered with the DVLA to access driver data as a controller, so we run the check in our own name and share the result with [Company Name] as a separate data controller for its own use of the result.

Driver Codes will obtain your driving record using one of two methods, depending on which is operationally available at the time of each check. By signing this authority, you are authorising Driver Codes to use either method.
Driver Codes generates a DVLA share code using the View Driving Licence service, acting under the authority you are signing here, and uses that share code to retrieve your driving record information from DVLA. Where you have linked your DVSA CPC training account, Driver Codes also retrieves your CPC training data.
Where the concierge workflow is not available or cannot complete, Driver Codes submits an enquiry under the DVLA Access to Driver Data (ADD) service. DVLA discloses your driving record from the computerised driver register to Driver Codes, as data controller, for the purpose of providing the driver compliance service to [Company Name].
Driver Codes decides which method to use for each check. Both methods are used solely for the purpose described in the previous step. Driver Codes will request your information as and when required by [Company Name] for the duration of this authority.
This authority will remain valid until you are no longer employed or engaged to drive for [Company Name], or in any case no more than three years from the date you sign this declaration — whichever happens first.
If [Company Name] needs continued access after that date, you will be asked to sign a new authority.
You can revoke this authority at any time through the ‘Company checks’ section of the Driver Codes app. [Company Name] will be notified when you do so, and no further checks will be carried out using this authority. Revocation applies going forward only; it does not affect checks already completed.

The driver reads the following declaration in full before continuing:
I confirm that I am the driver named in this request from [Company Name], that I am aged 18 or over, and that I am signing this authority of my own free will.
I authorise RSMT Limited (trading as Driver Codes) to obtain my driving record information from DVLA, and where I have linked my DVSA CPC training account my CPC training information from DVSA, for as long as this authority is valid. I understand that Driver Codes may use either:
(a) the Driver Codes concierge workflow — generating a DVLA share code via the View Driving Licence service and using it to retrieve my driving record; and/or
(b) the DVLA Access to Driver Data (ADD) service — under which DVLA will disclose my driving record to Driver Codes from the computerised driver register.
The information obtained may include: personal details; driving entitlements (with any associated restriction codes); valid endorsements; disqualifications (if any); photo images (where available); Certificate of Professional Competence (CPC) details; and Digital Tachograph Card details.
Driver Codes will share this information with [Company Name] for the purpose of verifying and monitoring my entitlement to drive in connection with my employment or engagement. RSMT Limited (trading as Driver Codes) and [Company Name] each act as separate, independent data controllers for their own processing. I have been given access to the Driver Codes Driver Checks Privacy Notice.
This authority will expire when I am no longer employed or engaged to drive for [Company Name], or in any event no later than three years from the date of my signature — whichever is sooner.
I may revoke this authority for future checks at any time via the ‘Company checks’ section of the Driver Codes app, and [Company Name] will be informed. Revocation affects future checks only and does not require deletion of records that Driver Codes is entitled to keep.
I understand that it may be a criminal offence to make a false statement in this declaration, or knowingly to obtain another person's personal data without their authority.
The driver is shown their details pre-filled from their DVLA record, then asked to sign using their finger or a stylus.

If any of the above details are incorrect or out of date, you must update DVLA before proceeding. By law, you must tell DVLA immediately if you change your permanent address or name. You could be fined up to £1,000 for failing to do so.

Once the driver taps Accept, the mandate becomes active and [Company Name] is able to run periodic licence checks through Driver Codes for the duration of the authority (up to three years). The driver can view and revoke active mandates at any time from the Company checks section of the app.
